*Diving Fitness Certificate
To participate in a guided dive trip, it is important that you are in good health. Although certain health issues do not fundamentally exclude diving, these should be examined by a doctor beforehand. To avoid diving accidents, it is required that you provide a certificate of your fitness to dive before the trip. This can take the form of a medical questionnaire (self-disclosure) or a medical certificate.
You can download the medical questionnaire from our website and fill it out independently. The completion is done at your own responsibility and must be signed by you. The certificate is valid for one year from the date of issue, or until your health status changes (whichever comes first). Should you have to answer a question in the questionnaire with „Yes“, you may need additional medical certification. A precise guide for filling it out can be found directly on the formplease pay particular attention to the notes in the fine print at the end of the form!
We recommend providing a medical certificate and regularly undergoing a fitness-to-dive examination for your own safety. According to the Diving and Hyperbaric Medicine Society, divers aged 8 to 15 years and those over 40 years should have a diving medical examination annually. Divers between 16 and 39 years should have examinations every three years or when their health status changes.
